You now know that you need to take care of yourself or your loved one, one of the best things you can do is organize your information.  This will give you the confidence and peace of mind knowing where all of your information is.  It will also help anyone who is attending to you or your loved one get to the important information quickly and easily.

You may be feeling exhausted or angry:  “Why is this happening to me?”

Unfortunately, I don’t have an answer for that.

I do know that it takes a lot of courage and perseverance to get through each day.

You can smile at the end of each day knowing you did the best you could!

If you or your loved one has a chronic illness, document and organize the following information:

  • Your Key Information
  • Appointments
  • Doctors and Caregivers
  • Emergency Procedures and Contacts
  • Special Equipment
  • Prescriptions
  • Vitals and Medications
  • Receipts and Insurance Records

If you are caring for an elder or special needs person, in addition to the above, you’ll need:

  • Home Safety Assessment
  • Caregiving Needs Assessment
  • Meal Planning
  • Grocery Shopping and Other Services

Perhaps you have a professional caregiver or multiple caregivers who are providing in-home care.  In this case, it is very important to document what happened and the information communicated with the next caregiver who is taking over.

If you are a family caregiver, I highly recommend you take a few minutes at the end of the day to document what happened during the day or night.  It’s very easy to confuse when something happened or remember what happened several days or weeks later.
